Welcome, New Patients!

Tennessee Foot & Ankle Clinic, a private podiatry practice located in Lenoir City, Madisonville, and Sevierville, Tennessee, welcomes all new patients to each of our locations. Please read the following information on scheduling appointments, new patient forms, what to bring to your appointment, and payments.

SCHEDULING APPOINTMENTS

Please call us at (865) 986-2700 to make an appointment. If you are unable to keep your appointment, please call us as far in advance as possible so we can use that time to see another patient in need of care.

We make a sincere effort to adhere to our appointment schedule and appreciate your patience if we are late due to emergencies or hospital surgery.

WHAT TO BRING

We ask that all new patients plan on arriving for their appointment early. It is important that you bring the following to your appointment:

  • Insurance card(s)

  • Picture ID, showing name and current address

  • Completed New Patient Forms

  • Referral forms (if required by insurance)

  • Any required co-payment

  • X-ray films/CD, lab work, MRI/Bone Scan

  • List of all prescription and/or over-the-counter medications that you are taking

  • Pertinent medical and surgical history information

We ask our patients to complete New Patient Forms before their first appointment. Please click on the link below, print the forms, and fill them out before coming for you first visit.

FEES & PAYMENT

We make every effort to decrease the cost of your medical care. Therefore, we request payment arrangements for all office services at the time they are rendered unless prior arrangements have been made.

 

We accept cash, checks, MasterCard, Visa, and Discover for your convenience.

If we are a participating provider of your insurance company, we will bill them. However, payment is the patient’s responsibility. We will help in any way we can to assist you in handling claims.
